Most Recent Tax Years and Income:  Call the IRS at 1-800-829-1040 and ask them to fax or mail the most recent tax transcript. We will need this BEFORE we can file. Note: If we are filing after April 15 of a given year, you had to have filed your income taxes for the preceding year.  Filing an extension is not enough for United States Citizenship and Immigration Service.

We also need the sponsor’s W-2 or 1099 form.  Additionally if a joint sponsor is involved, we need their proof of US Citizenship (birth certificate or citizenship certificate) or green card and their W-2 and current job letter.

Information needed for I-130/I-485 for the Petitioner and all Beneficiary/ies (the person you want to sponsor). Please fill out one form for each person.

  • Names and address of all employers for the last 5 years.
  • What position you worked, for ex. Shell Oil, 1234 Any Place, Houston, TX as Project Engineer, from May 97 to June 2000.
  • Name of father, Father’s date of birth, Where he was born (city, country) and where he is now (City and country)
  • Name of Mother, and maiden name, Mother’s date of birth, Where she was born (city, country) and where she is now (City and country)
  • If beneficiary is in the U.S., include a copy of his/her passport, visa status and I-94
